For PTSA Reimbursements:

Download and fill out the PTSA Reimbursement form. Please sign the form and gather the receipts that you need to get reimbursed for. To submit:

1. You can attach the receipts to the form and turn it into the PTSA mail box in the MVHS office mailroom.
2. Scan/take pic of your receipts and the signed form and email them to mvhs.ptsa.treasurer@gmail.com. Please allow 2 weeks for your reimbursement to arrive. Contact the email above if you have any questions.

We suggest you keep copies of the reimbursement form and receipts for your records.
